AFFIDAVIT TO REPORT A MISSING PARKING TAG

File this affidavit upon discovering a parking tag missing. Fill in all information or the affidavit will not process.

I attest that my assigned parking tag is missing and I did not authorize anyone access to it. I request that University Police render this tag invalid, effective immediately, to protect me from financial liability due to unauthorized use. I understand that I remain liable for all fines or fees imposed prior to this filing date.

A parking tag that is reported missing, and subsequently used, violates the campus Traffic and Parking Regulations, subjecting a violator to disciplinary action and towing, without warning. If I find the missing tag, I will immediately return this tag to: UPD, 101 Broad Street (Health Center), Plattsburgh, NY 12901.
