Disciplinary Action: Suspension is defined as a sanction imposed for disciplinary reasons that results in a student leaving school for a fixed period, less than permanently. Dismissal from a college for disciplinary reasons is defined as permanent separation from an institution of higher education on the basis of conduct or behavior not on academic performance.
An affirmative response will not automatically prevent admission, but you will be asked to provide additional information. Any falsification or omission of data may result in denial of admission or dismissal.

Your application will be reviewed upon our receipt of all required information from you. The Admissions Office will contact you if additional information is needed to review your application for admission. Any missing items will be posted online in your MyPlattsburgh account. Completion of this form alone does not guarantee admission. The outcome of this review will be mailed to you at the address you have indicated on this form. TERMS AND CONDITIONS
Cardinal Pathway Participants agree to the following:
1. Take a minimum of 24 credit bearing credits at another accredited two or four year college or university. Remedial courses do not count toward the total credits earned.
2. Earn a minimum of a 2.3 cumulative grade point average. Students seeking to enter the following majors Education (all programs), Business (all programs), Nutrition, Communication Sciences and Disorders, Human Development and Family Relations (all programs), Fitness & Wellness Leadership, Social Work, or Criminal Justice as a transfer student may do so only if they meet the grade point average required of students applying for transfer admission to these programs. 3. Provide the college or university that will be attended and the courses that will be taken in the interim, mid-term grades during the fall and spring semesters, final fall grades, and speak with a transfer advisor about academic progress.
4. Provide a final official transcript at the end of the academic year.
5. The understanding that Nursing will not be an option for the enrolling students at a later point in time.
6. Transfer admission into EOP is reserved for those students who were previously enrolled in an EOP-type program (such as EOP, HEOP, SEEK, or College Discovery).
